And sometimes, that is part of the story. Hormonal shifts can affect sleep, mood, memory and concentration. It is biological.<\/p>\n<p>Estrogen and dopamine work closely together in the brain, says <a rel=\"noreferrer noopener nofollow\" data-id=\"https:\/\/www.adhdpowerbank.com\/about-sandra-kooij\/\" target=\"_blank\" data-type=\"link\" href=\"https:\/\/www.adhdpowerbank.com\/about-sandra-kooij\/\">Dr. Sandra Kooij<\/a>, a psychiatrist, professor of adult ADHD at Amsterdam University Medical Center and co-founder of the <a rel=\"noreferrer noopener nofollow\" data-id=\"https:\/\/www.h3-netwerk.nl\/\" target=\"_blank\" data-type=\"link\" href=\"https:\/\/www.h3-netwerk.nl\/\">Head Heart Hormones (H3) Network<\/a> Foundation. Estrogen plays a role in dopamine production and also helps inhibit its breakdown. Because dopamine is involved in focus, motivation, memory and mood regulation, <a data-id=\"2955075\" data-type=\"post\" href=\"https:\/\/www.theflowspace.com\/reproductive-health\/menopause\/estrogen-matters-update-avrum-bluming-sharon-malone-alloy-health-2955075\/\" rel=\"nofollow noopener\" target=\"_blank\">falling estrogen levels<\/a> can hit some women especially hard.<\/p>\n<p>\u201cWhat we think is happening in women with ADHD during perimenopause is thus a \u2018double hit\u2019: low or dysregulated dopamine related to ADHD, plus decreasing levels of estrogen,\u201d Kooij says. \u201cThis may explain why these women experience more severe perimenopausal cognitive and mood symptoms.\u201d<\/p>\n<p>Perimenopause, then, may not cause ADHD \u2014 but it can expose symptoms that were once easier to mask or manage.<\/p>\n<p>The Biggest Clue? Timing<\/p>\n<p>The clearest difference between ADHD and perimenopause-related <a href=\"https:\/\/www.theflowspace.com\/t\/brain-fog\/\" id=\"auto-tag_brain-fog\" data-tag=\"brain-fog\" rel=\"nofollow noopener\" target=\"_blank\">brain fog<\/a> is when the symptoms began.<\/p>\n<p>ADHD is a neurodevelopmental condition, meaning signs begin in childhood, even if they were missed, minimized or masked. Perimenopause-related cognitive changes usually emerge later, often alongside other hormonal symptoms.<\/p>\n<p>Kooij says women with ADHD typically have had concentration and memory issues throughout life that worsen during perimenopause. Women without ADHD may experience may experience brain fog, forgetfulness or concentration issues for the first time during the menopausal transition.<\/p>\n<p>Hamdani points to a similar distinction. Symptoms that persist across settings and over years \u2014 at work, at home, in relationships and daily routines \u2014 may suggest ADHD, especially if there is a family history or an earlier pattern of inattention, disorganization or emotional dysregulation.<\/p>\n<p><a href=\"https:\/\/www.theflowspace.com\/wp-content\/uploads\/2026\/07\/H3-Womens-Care-Infographic-English.png\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><img loading=\"lazy\" decoding=\"async\" height=\"724\" width=\"1024\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/07\/H3-Womens-Care-Infographic-English.png\" alt=\"Head, Heart and Hormones infographic on ADHD and women's hormones\" class=\"wp-image-3037500\"  \/><\/a>     Courtesy of the Head Heart Hormones (H3) Network Foundation<\/p>\n<p>It May Be ADHD If \u2026<\/p>\n<p>You can trace similar struggles back years or decades, even if you were able to compensate.<\/p>\n<p>That may look like chronic procrastination, time blindness, losing things, difficulty finishing tasks, emotional reactivity, intense overwhelm, impulsive spending, interrupting, overcommitting or feeling like everyday life takes more effort for you than for other people.<\/p>\n<p>For high-achieving women, ADHD may not read as obvious chaos from the outside. It may look like overpreparing, people-pleasing, perfectionism, working late to catch up or using anxiety as a productivity tool.<\/p>\n<p>Then perimenopause arrives. Sleep worsens, estrogen fluctuates and the systems that once kept everything together may stop working.<\/p>\n<p>It May Be Perimenopause If \u2026<\/p>\n<p>The symptoms feel new, clustered in midlife and appear alongside other signs of hormonal change. That may include irregular periods, hot flashes, night sweats, sleep disruption, joint pain, urinary symptoms, heart palpitations, migraines or mood changes that feel tied to your cycle.<\/p>\n<p>Perimenopause can also worsen anxiety, irritability and low mood, which can make focus and memory feel worse \u2014 even without ADHD.<\/p>\n<p>The challenge is that many women do not arrive at the doctor with one neat complaint. They arrive with a pileup: poor insomnia, rage, forgetfulness, overwhelm, fatigue and the sense that they no longer recognize themselves.<\/p>\n<p>It Can Absolutely Be Both<\/p>\n<p>For those who already know they have ADHD, perimenopause can intensify symptoms. For those who were never diagnosed, it can reveal traits that were previously hidden by structure, coping strategies or sheer willpower.<\/p>\n<p>Kooij says women with ADHD whose symptoms increase during this stage may describe losing control over mood, irritability, sleep and focus. In some cases, the impact can be severe enough to affect work.<\/p>\n<p>That is why dismissing these symptoms as \u201cjust stress\u201d or \u201cjust hormones\u201d can leave women without the right care.<\/p>\n<p>\u201cThis is a very real phenomenon, and women deserve to be heard and treated,\u201d Hamdani says.<\/p>\n<p>What to Track Before You Seek Help<\/p>\n<p>Before seeing a clinician, keep a simple log for several weeks. Note when symptoms started, whether they fluctuate with your cycle, how your sleep is changing and where the biggest disruptions show up \u2014 work, home, relationships or all of the above.<\/p>\n<p>Also write down any lifelong patterns: school struggles, chronic disorganization, emotional intensity, family history of ADHD or years of feeling like you were barely keeping up.<\/p>\n<p>Bring specific examples, not just \u201cI can\u2019t focus.\u201d Try: \u201cI\u2019m missing deadlines I used to meet,\u201d \u201cI can\u2019t remember conversations,\u201d \u201cI\u2019m losing my temper in ways that feel unlike me\u201d or \u201cmy coping systems stopped working.\u201d<\/p>\n<p>The goal is not to diagnose yourself.
