These therapies use a modified, harmless virus to deliver therapeutic genes directly to diseased cells, helping them to function normally again.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">But numerous clinical trials have found that gene therapy causes <a href=\"https:\/\/www.frontiersin.org\/journals\/immunology\/articles\/10.3389\/fimmu.2020.00670\/full\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:serious side-effects in some patients;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">serious side-effects in some patients<\/a> because their immune system recognises the virus and attacks it.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">So to improve safety and efficacy of gene therapy in the eye, my colleagues and I wanted to understand more about this immune response so we can someday prevent it from happening.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">We discovered that old female mice were more vulnerable to experiencing a <a href=\"https:\/\/www.cell.com\/molecular-therapy-family\/molecular-therapy\/fulltext\/S1525-0016(25)00032-2?_returnURL=https%3A%2F%2Flinkinghub.elsevier.com%2Fretrieve%2Fpii%2FS1525001625000322%3Fshowall%3Dtrue\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:damaging side-effect from gene therapy;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">damaging side-effect from gene therapy<\/a>, compared to both male mice and young female mice. This reaction was directly related to differences in the way the immune systems of old female mice functioned.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">To conduct our study, we gave both male and female mice the gene therapy, which was delivered into the eye. The therapy was tested on young, middle-aged and old mice.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">We found that in young mice, females had increased immune activation \u2013 even from a lower dose of gene therapy. A <a href=\"https:\/\/www.nature.com\/articles\/s41434-025-00528-7\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:similar finding;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">similar finding<\/a> was also recently observed in human blood samples \u2013 with women\u2019s immune cells exhibiting greater amounts of inflammation, a sign these immune cells were mounting an attack against the therapy.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Our research went on to show that age was also associated with a stronger inflammatory response to the gene therapy. This was true for both old male and female mice. The inflammatory response lasted longer in the older mice, too.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">When we looked more closely at a specific type of immune cell that\u2019s found in the brain and eye, we saw that in older female mice these cells showed signs of both an earlier stress response and stronger inflammatory reaction compared to younger mice and male mice. This reaction was also linked to signs of tissue degeneration.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Together, these findings suggest that women, particularly older women, could be at greater risk of harmful reactions to gene therapy \u2013 especially at the doses needed for these therapies to work.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Although our study was conducted in mice, it\u2019s not the first research to show that immune response differences can affect the way men and women react to certain treatments.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Another research group also showed that female mice <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36293546\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:mounted a stronger response;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">mounted a stronger response<\/a> against a gene therapy \u2013 recognising it as foreign and removing it. This reduced the amount of therapy delivered successfully to females compared to males.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Treatments for conditions such as arthritis are another example of the way women\u2019s immune response can affect how they respond to treatment. These immunotherapies <a href=\"https:\/\/bsd.biomedcentral.com\/articles\/10.1186\/s13293-020-00301-y\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:work better in men compared to women;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">work better in men compared to women<\/a>. Some scientists believe this is because women\u2019s immune systems are more likely to recognise the drug <a href=\"https:\/\/www.tandfonline.com\/doi\/10.1080\/03009742.2019.1602670?url_ver=Z39.88-2003&amp;rfr_id=ori:rid:crossref.org&amp;rfr_dat=cr_pub%20%200pubmed\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:as foreign and remove it;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">as foreign and remove it<\/a>.<\/p>\n<p>Sex differences and immune function<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">There\u2019s one key reason men\u2019s and women\u2019s immune systems may respond differently to the same treatment. Women\u2019s immune systems are <a href=\"https:\/\/www.nature.com\/articles\/nri.2016.90\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:generally more reactive;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">generally more reactive<\/a> than men\u2019s to anything unfamiliar.<\/p>\n<p><img alt=\"A digital drawing depicting a strand of DNA, alongside the X chromosome.\" loading=\"lazy\" width=\"754\" height=\"431\" decoding=\"async\" data-nimg=\"1\" class=\"rounded-lg\" style=\"color:transparent\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2025\/08\/d3299e79c8b832430ac9eb0d25b2be8f.jpeg\"\/><\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">The types of immune cells that respond the strongest and fastest to a foreign substance are different for men and women. This is because sex hormones \u2013 primarily oestrogen for females and testosterone for males \u2013 <a href=\"https:\/\/www.nature.com\/articles\/nri.2016.90\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:directly affect the way these cells behave;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">directly affect the way these cells behave<\/a>.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">The X chromosome also plays a role in immune response, as it contains a greater number of immune-related genes compared to the Y chromosome. Females have two X chromosomes, while males have one X and one Y chromosome. This leads to a persistent inflammatory state, which is thought to be involved in <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0278584622000689\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:many age-related conditions;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">many age-related conditions<\/a> \u2013 including cardiovascular disease and neurodegeneration.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Differences in immune cell function also <a href=\"https:\/\/www.nature.com\/articles\/s41467-020-14396-9\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:become more evident;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">become more evident<\/a> for men and women after the age of 65.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">In older men, their inflammatory immune cells are more active after the age of 65, while their adaptive immune cells (which coordinate the recognition and removal of foreign pathogens) <a href=\"https:\/\/www.nature.com\/articles\/s41467-020-14396-9\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:are less active;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">are less active<\/a>.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">But as women age, their <a href=\"https:\/\/www.nature.com\/articles\/s41467-020-14396-9\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:adaptive immune cell activity;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">adaptive immune cell activity<\/a> can stay the same. Having a stronger adaptive immune response increases the risk of autoimmune conditions \u2013 a condition that has been linked to <a href=\"https:\/\/theconversation.com\/alzheimers-might-not-be-primarily-a-brain-disease-a-new-theory-suggests-its-an-autoimmune-condition-189047\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:certain neurodegenerative diseases;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">certain neurodegenerative diseases<\/a>.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">These differences help explain why vaccines are <a href=\"https:\/\/www.frontiersin.org\/journals\/aging\/articles\/10.3389\/fragi.2022.836642\/full\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:less effective for older men;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">less effective for older men<\/a> because they have fewer functioning adaptive immune cells. They may also help to explain why the older female mice in our study had adverse reactions to the gene therapy, as their immune cells are more primed to attack.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Women have historically been underrepresented <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9043984\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:in biomedical research;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">in biomedical research<\/a>. Modelling everything on males has <a href=\"https:\/\/www.understandinganimalresearch.org.uk\/news\/why-we-need-female-mice-in-drug-trials\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:created a data gap;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">created a data gap<\/a> in clinical translation, causing harm. For instance, women have nearly <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7275616\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:twice as many adverse reactions;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">twice as many adverse reactions<\/a> to a drug compared to men.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">But studies like ours are helping to provide important insight into why certain medications are less effective in women \u2013 and how the immune system is implicated.
