In September, 120 funding awards for HIV\/AIDS work carried out by the US Centers for Disease Control are set to expire, with no concrete replacement system in place.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000p3b6r9odzvsj0@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The programs provide services to more than 8.7 million patients worldwide, analysts say, and it\u2019s unclear what will happen to many patient services on the other side of October 1.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000q3b6rlzp2u7nr@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            It comes as the US State Department is restructuring the CDC\u2019s work on global health initiatives to assume greater control, according to internal guidance published by the State Department in May, of which CNN obtained a copy.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000r3b6rdrmtce8q@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The new guidance lays out a \u201cstreamlined\u201d approach to the United States\u2019 long-running HIV\/AIDS initiative, called the President\u2019s Emergency Plan for AIDS Relief (PEPFAR), which was established by the Bush administration in 2003. Considered a flagship among global health initiatives, PEPFAR is credited with <a href=\"https:\/\/www.kff.org\/global-health-policy\/the-u-s-presidents-emergency-plan-for-aids-relief-pepfar\/\" target=\"_blank\" rel=\"nofollow noopener\">saving more than 26 million lives<\/a> and preventing millions of infections, mostly in Africa.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000s3b6rq9avwll1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Previously, PEPFAR was jointly run by the <a href=\"https:\/\/www.cnn.com\/2025\/03\/28\/politics\/state-department-formally-notifies-congress-effectively-dissolving-usaid\" rel=\"nofollow noopener\" target=\"_blank\">US Agency for International Development<\/a> (USAID), CDC and other agencies, and overseen by the State Department. But the new plan will swiftly move much more control to the State Department. Multiple critics and experts have told CNN that they support the idea of streamlining PEPFAR \u2013 work to improve its efficiency was already underway \u2013 but they believe this new approach will severely diminish the initiative\u2019s effectiveness and sideline the health experts at CDC.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000t3b6rohu7hoc6@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The 120 US-financed awards for the <a href=\"https:\/\/www.cnn.com\/2026\/05\/07\/world\/trump-administration-usaid-global-health-funding-intl\" rel=\"nofollow noopener\" target=\"_blank\">CDC branch of PEPFAR<\/a> are expected to end within weeks without replacement mechanisms in place, according to a recent analysis of publicly available data by the Health Security Policy Academy, a US-based think tank.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000u3b6rhwm2400y@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe result could be a second global health woodchipper: the abrupt destruction of operating systems that patients, clinics, health workers, laboratories, and ministries of health still depend on,\u201d wrote the authors of the analysis.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/07\/gettyimages-2212118972-20260708125302046.jpg\" alt=\"A mentor working as part of a program to prevent mother-to-child HIV transmission sits with a mother and her son at Migosi Sub-county Hospital in Kisumu, Kenya, in April 2025 \u2013 part of a wide range of work to combat HIV\/AIDS that is supported by CDC.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1333\" width=\"2000\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000v3b6rbh7jhqvx@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The impacts are expected to vary greatly from country to country. Mozambique, Tanzania and South Africa will be hit particularly hard by the funding cliff this year, <a href=\"https:\/\/globalhealthwatch.org\/?blog=pepfar-shutdown\" target=\"_blank\" rel=\"nofollow noopener\">the analysis shows<\/a>.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000w3b6r47t31e37@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The awards fund a range of services including community testing, HIV clinical services, HIV lab services, Pre-Exposure Prophylaxis (PrEP) medication and more. Many of the expiring overseas programs have been running for 15 or 20 years, since the funding was typically renewed in five-year cycles.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i2z000x3b6rvhg23en1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In addition to not renewing US-funded programs that happen to expire in 2026, the State Department is also planning to replace some of CDC\u2019s other remaining awards and has told CDC to cancel some open funding opportunities, according to one CDC official.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30000y3b6rwth4ppg5@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cIt really does feel like the end of PEPFAR,\u201d the CDC official, who was not authorized to speak on the record, told CNN. \u201cIn a lot of cases, the State Department mechanism is not set up yet. So, this isn\u2019t handing it from one set of experts to another \u2013 this is taking it away from one set of experts and putting it in a big box with a question mark.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30000z3b6rpcjdwgou@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Another part of the new strategy launched in May is transitioning to a system in which partner countries choose from a \u201cmenu\u201d of services, paying \u00e0 la carte for which CDC services they want. Analysts say that will take the US health agency out of the driver\u2019s seat and turn it into a contractor of sorts.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000103b6r2guejr10@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Trump administration political appointees \u201chave been pretty clear the overall funding is going way down,\u201d the CDC source added, saying that State Department officials have made clear that they are no longer interested in funding certain systemic work, such as health surveillance systems, certain laboratory work and health workforce training. For the parts of PEPFAR programs that they do want to keep funding, like health workers and medications, \u201cit\u2019s really just for one or two more years, and then they\u2019ve already told some of the countries we\u2019re pulling the plug.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000113b6rtvx3oyni@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The State Department though has said that funding for operations will increase, as it transitions to what it\u2019s calling the <a href=\"https:\/\/www.cnn.com\/2025\/11\/26\/politics\/trump-global-health-strategy-intl\" rel=\"nofollow noopener\" target=\"_blank\">\u201cAmerica First Global Health Strategy.\u201d<\/a> The changes to PEPFAR are part of that broader strategy, in which American aid will be funneled through a new system of one-on-one Memoranda of Understanding (MOU) with individual countries, rather than distributed through international aid partners and organizations.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000123b6r8h6idmfm@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe State Department expects CDC overseas operating funding to increase \u2013 not decrease \u2013 under the America First Global Health Strategy,\u201d a State Department spokesperson said in response to questions from CNN.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/07\/gettyimages-2224118204.jpg\" alt=\"A cyclist rides past a PEPFAR sign in Abidjan, Ivory Coast, in July 2025.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1333\" width=\"2000\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000133b6r0w8oi4ez@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe Trump Administration is preserving and strengthening PEPFAR\u2019s lifesaving impact, maintaining CDC\u2019s world-class technical role, and ensuring US assistance is producing durable results,\u201d the State Department spokesperson said. They said that each country receiving aid is now working through a plan for implementation of the MOU agreements and that the CDC remains \u201cthe preferred provider for all technical services to recipient countries.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000153b6ri4cz9lnj@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A spokesperson for the US Department of Health and Human Services, which oversees CDC, said: \u201cThe suggestion that the America First Global Health Strategy will \u2018deteriorate\u2019 global health programs ignores decades of CDC\u2019s work to build sustainable public health capacity around the world,\u201d adding that \u201cCDC continues to implement global health programs funded through Congressional appropriations.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000173b6ryp2hz3zu@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The State Department\u2019s restructure of PEPFAR has drawn widespread criticism from experts, members of Congress and former CDC officials who worked under both Republican and Democratic administrations.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000183b6rxnnc295k@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In an essay published in the health news outlet Stat in May, <a href=\"https:\/\/www.statnews.com\/2026\/05\/26\/pepfar-state-department-plan-dismantling-hiv-cdc\/\" target=\"_blank\" rel=\"nofollow noopener\">eight former CDC directors warned<\/a> that the State Department strategy, \u201cdone in haste as currently planned\u2026 will destroy PEPFAR and undermine health security around the world and in our own country.\u201d They added that the fee-for-services menu \u201crisks pulling out the scaffolding and expertise\u201d of the program before an effective alternative is in place.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i3000193b6rqq2uq9tx@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The former CDC directors urged the State Department, CDC and Congress to work together on a more stable transition plan.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30001a3b6ry6w34t2z@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Meanwhile, a bipartisan group of senators has sent a <a href=\"https:\/\/www.warnock.senate.gov\/wp-content\/uploads\/2026\/06\/SRW_Letter-on-PEPFAR.pdf\" target=\"_blank\" rel=\"nofollow noopener\">letter<\/a> to the Senate Appropriations Committee urging a direct increase in funding for CDC\u2019s global health account, arguing that \u201cwhile we must be conscientious stewards of taxpayer dollars spent on foreign assistance, PEPFAR is one of the most cost-effective foreign assistance programs, delivering an immense return on investment.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30001b3b6rmbwo261t@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            CDC has a \u201cproven track record\u201d of successfully implementing the US\u2019 global work on HIV\/AIDS, said the group of 23 senators, led by the Rev. Raphael Warnock (D-GA) and Dr. Bill Cassidy (R-LA).\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30001c3b6rcj74c64k@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cA real challenge with this proposal is that it\u2019s really viewing global HIV work and global health-response work as transactional, as part of these MOU deals,\u201d said Dr. Michele Montandon, the former leader of the CDC team working to prevent mother-to-child transmission of HIV, who was laid off last year. \u201cThere\u2019s really a lack of focus on health outcomes, saving lives, keeping Americans safe.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30001d3b6rxl4dgh29@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Montandon expressed concerns about the continuation of the accountability, transparency and strong data collection that have long been considered the keys to PEPFAR\u2019s success, as the new system of bilateral MOU deals is implemented.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/07\/gettyimages-2200213343.jpg\" alt=\"An AIDS support counsellor speaks to clients during a clinic day in February 2025 in Kampala, Uganda, as part of a range of services supported by CDC.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1333\" width=\"2000\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30001e3b6racyu3eyp@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cWhen you require countries to meet certain indicators, we know that people are more likely (to) tell you what you want to hear. And so you need a lot of data quality systems in place, and site visits, and oversight to make sure that the data are accurate,\u201d she told CNN.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30001f3b6ri5v96zrp@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Regarding the expiring awards, Montandon added: \u201cIt\u2019s really hard to transition the support for 8 million people in a couple months\u2019 time without having major disruptions\u2026 When people lose their access to treatment, they get sick with AIDS. Babies get HIV. People die.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmrc81i30001g3b6rhyux1dyh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The CDC official added that staff inside the agency have long been working to improve PEPFAR\u2019s efficiency and bolster the involvement of partner governments, in order to help ensure that the money the United States spends to combat HIV\/AIDS goes further.
