The former child star died on Feb. 8 at age 33<\/a> from an accidental fentanyl overdose, according to the Oklahoma Office of the Chief Medical Examiner\u2019s report obtained by TMZ<\/a> Monday.<\/p>\n Per the report, he was living in a sober living home located in Tulsa, Oklahoma.<\/p>\n \u201cHow to Eat Fried Worms\u201d star Blake Garrett\u2019s cause of death has been revealed. FilmMagic<\/p>\n Garrett was rushed to the hospital and diagnosed with shingles the week before his death, his mother, Carol, previously told the outlet.<\/p>\n At the time, the matriarch feared her son\u2019s death might have been accidental, speculating that he might have been self-medicating.<\/p>\n The CDC defines shingles<\/a> as a \u201cpainful rash,\u201d which is \u201ccaused by varicella-zoster virus (VZV), the same virus that causes chickenpox.\u201d<\/p>\n He had been living in a sober living home located in Tulsa, Oklahoma.
