I told one person and <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1211\/096176702776868415?casa_token=CvpkWPG30WYAAAAA:aWNzy9l_alvSSA3_XQwPcDFpuV1tppmKNbGfjIIP6lLVEBEgnAVVRH9X6V223zcvo68MpGJPZ7ZeXNA\" rel=\"nofollow noopener\" target=\"_blank\">now he doesn\u2019t talk to me anymore<\/a>.\u00ab<\/p>\n<p>These are the words of a woman based in Copenhagen. You would be forgiven for thinking she is referring to something shameful or unacceptable. In fact, she is talking about her use of antidepressant medication.<\/p>\n<p>However this was, admittedly, more than 20 years ago.\u00a0<\/p>\n<p>Have things changed since then? Do we now live in a world where people feel comfortable, should they so choose, to discuss their use of antidepressants in the same way as other medications?<\/p>\n<p>Unfortunately, this does not seem to be the case. <a href=\"https:\/\/www.mdpi.com\/1660-4601\/19\/24\/16788\" rel=\"nofollow noopener\" target=\"_blank\">More recent studies<\/a> show that people <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/nmo.13956?casa_token=L05qtq9wtagAAAAA%3AAJSudDBAaHWXm-wsQvwoFpeM3OJGFRqX4qFDfv2vvAWqF4qTlegA8tDbz8nOTlItrdX-S6AzsKwFHQU\" rel=\"nofollow noopener\" target=\"_blank\">feeling embarrassed<\/a> about taking antidepressants makes it harder for them to <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0165178123006145?casa_token=DKpzwDYq9aIAAAAA:R5uayC7pTI34bWngJZnpPa2qDduqOarenkMKnJPqZ_R3dgERncNGyUekXs7T8r4CVEFq3xugqf4\" rel=\"nofollow noopener\" target=\"_blank\">stick to their treatment<\/a>.\u00a0<\/p>\n<p>People still expect judgement if they choose to open up about their medication use:\u00a0<\/p>\n<p>\u00a0\u00bb I just thought I can\u2019t be bothered being judged for it,\u00ab <a href=\"https:\/\/www.mdpi.com\/1660-4601\/19\/24\/16788\" rel=\"nofollow noopener\" target=\"_blank\">said one user in a 2019 study<\/a>.<\/p>\n<p>The studies I just quoted are not Danish. But a 2023 review shows that stigma towards the mentally ill is <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35930387\/\" rel=\"nofollow noopener\" target=\"_blank\">no less in the Nordic countries than the rest of the world<\/a>.<\/p>\n<p>Antidepressants are common, so why are they taboo?\u00a0<\/p>\n<p>An astounding <a href=\"https:\/\/psykiatrifonden.dk\/viden\/fakta-forskning\/tal-fakta-om-psykisk-sygdom-danmark-2023\/forekomst-psykisk-sygdom\" rel=\"nofollow noopener\" target=\"_blank\">82 percent of the Danish population<\/a> will receive treatment for mental ill health at some point in their lives. Depression is the <a href=\"https:\/\/link.springer.com\/content\/pdf\/10.1007\/s41669-019-0167-7.pdf\" rel=\"nofollow noopener\" target=\"_blank\">fourth most common<\/a> of 199 chronic conditions in Denmark.<\/p>\n<p>Antidepressant use has <a href=\"https:\/\/www.euronews.com\/health\/2023\/09\/09\/europes-mental-health-crisis-in-data-which-country-uses-the-most-antidepressants\" rel=\"nofollow noopener\" target=\"_blank\">more than doubled<\/a> in Europe over the past two decades. Given the pervasiveness of the disorder, it is likely that you know someone suffering from depression.<\/p>\n<p>So why are people afraid to share that they take antidepressants?<\/p>\n<p>One line of thinking points to the feeling that taking them is <a href=\"https:\/\/journals.lww.com\/jonmd\/abstract\/2011\/11000\/perceptions_of_and_attitudes_toward.9.aspx\" rel=\"nofollow noopener\" target=\"_blank\">an indication that your mental illness is severe<\/a>.\u00a0<\/p>\n<p>Alternatively, people may be concerned about being perceived as <a href=\"https:\/\/journals.lww.com\/jonmd\/abstract\/2011\/11000\/perceptions_of_and_attitudes_toward.9.aspx\" rel=\"nofollow noopener\" target=\"_blank\">emotionally weak and unable to deal with difficulties<\/a>.\u00a0<\/p>\n<p>Indeed, the increasing prescription of antidepressants has been argued to represent the <a href=\"https:\/\/www.theguardian.com\/society\/2013\/nov\/21\/prescribing-culture-blame-rise-antidepressants\" rel=\"nofollow noopener\" target=\"_blank\">over-medicalisation of difficult emotional states;<\/a> perhaps people feel they cannot share their medication use for fear of judgement that they are unable to deal with \u2018normal\u2019, human emotions.<\/p>\n<p>People want to \u2018kick the habit\u2019\u00a0<\/p>\n<p>Whilst these are valid and plausible explanations, they are intrinsically tied up with the more general stigmatisation of depression.\u00a0<\/p>\n<p>So what is it specifically about medication for depression?<\/p>\n<p>Conversations with individuals taking antidepressants often reveal concerns about their moral acceptability, with some discussions suggesting they are viewed <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0277953615301568#sec5\" rel=\"nofollow noopener\" target=\"_blank\">as somewhat illicit<\/a>.\u00a0<\/p>\n<p>The aforementioned perception that depression is itself illegitimate extends to their treatment; people with the condition have equated their antidepressant use with <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0277953615301568#sec5\" rel=\"nofollow noopener\" target=\"_blank\">the use of recreational drugs<\/a>.<\/p>\n<p>People <a href=\"https:\/\/bmjopen.bmj.com\/content\/5\/12\/e008636?cpetoc\" rel=\"nofollow noopener\" target=\"_blank\">are worried<\/a> about <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0165032714003711?casa_token=tQLXOe6jRUMAAAAA:8_D3-AWKoSW-H_F-3jHNx8jbvW49L077a8LMsdGvisnN6moGGWDBiS3Rvvi6BDtP3b0ZNav3JpE\" rel=\"nofollow noopener\" target=\"_blank\">drug dependency or \u2018addiction\u2019<\/a>, expressing a desire to <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0277953615301568?casa_token=WkFx__a_-jgAAAAA:irxfTqF1IeOA33R34zUkSrU_11TbemwQxRxe8e9jT6aXqBDqusiXsl9dNbfMz0o69eUoSQd4-0s\" rel=\"nofollow noopener\" target=\"_blank\">\u2018kick the habit\u2019<\/a>.\u00a0<\/p>\n<p>However, whilst this does get nearer to the crux of the matter, would these types of concerns be raised about medication for a somatic illness, like diabetes or hypertension? Probably not.\u00a0<\/p>\n<p>So why is medication for depression stigmatised?\u00a0<\/p>\n<p>Why are antidepressants seen differently to medication for somatic diseases?<\/p>\n<p>Antidepressants work by balancing the neurotransmitter levels in your brain. So perhaps we should turn our attention to the very foundations upon which antidepressants are based \u2013 that depression is caused, in part, by one\u2019s biology.\u00a0<\/p>\n<p>This is correct, however believing genetics is the sole cause has negative repercussions (I\u2019ll come back to this).<\/p>\n<p>A 2014 study of almost 2.000 people taking antidepressants found that the longer people remained on antidepressants and the more effective they found them, the more likely they were to believe that <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0165032714003711\" rel=\"nofollow noopener\" target=\"_blank\">biology plays a role in depression<\/a>.\u00a0<\/p>\n<p>Thus, the perceived efficacy of antidepressants goes hand-in-hand with a belief in the biological underpinnings of depression.<\/p>\n<p>However, the influence of genetics on psychiatric illnesses is not as simple as \u2018if you have the gene for depression, you will get depression\u2019.\u00a0<\/p>\n<p>Your genes play a role, yes, but they are not the sole actor. Everyone is born with a certain amount of genetic risk for psychiatric disorders, but <a href=\"https:\/\/www2.psych.ubc.ca\/~schaller\/528Readings\/HardenKoellinger2020.pdf\" rel=\"nofollow noopener\" target=\"_blank\">no one is biologically destined to experience one.<\/a>\u00a0<\/p>\n<p>Your environment is always a part of the story.<\/p>\n<p>Unfortunately, there has been <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/cge.12415?casa_token=mDhIFU-t-X8AAAAA%3ACIvCvHj-ewoi4Ipiza-MAo01n-5fJpRCbUYMvbxeNB5ZBDz1vE_bsRdqCFy77Ud4Z0dhFtC5bEWq_A\" rel=\"nofollow noopener\" target=\"_blank\">little progress<\/a> in successfully conveying this nuance to the general public.\u00a0<\/p>\n<p>Perhaps because of this, reviews have found that public endorsement of the biogenetic explanation of mental ill health (i.e. that depression has a genetic basis) is linked to <a href=\"https:\/\/www.cambridge.org\/core\/journals\/the-british-journal-of-psychiatry\/article\/biogenetic-explanations-and-public-acceptance-of-mental-illness-systematic-review-of-population-studies\/E6078EED07031B5828DB84FCDC90657F#article\" rel=\"nofollow noopener\" target=\"_blank\">wanting social distance from people with depression<\/a> and seeing them as <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0277953613004164\" rel=\"nofollow noopener\" target=\"_blank\">more dangerous<\/a>).<\/p>\n<p>Even amongst mental health professionals, <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/inm.12390?casa_token=d04HaZteTn8AAAAA%3A2VHaao4NggIeau1ElHswCScOMAwmu_eh68wKPfZ7ncTjLpb5o56_9166UeIRIHHQxMzIfHdDdWMFR9E\" rel=\"nofollow noopener\" target=\"_blank\">a review of the literature in 2017<\/a> found the majority of studies point to a link between biogenetic explanations of depression and less empathy towards those with the illness.<\/p>\n<p>We can speculate that this is because of a misunderstanding of the biology \u2013 believing genetics alone can determine mental health.<\/p>\n<p>Can we assume people understand the meaning of a bio-genetic explanation of depression?<\/p>\n<p>Indeed, we must interrogate what these studies mean by \u2018biogenetic\u2019 explanations.\u00a0<\/p>\n<p>The authors of the 2017 review stated that \u00bbthe majority of these studies gave little information or details of the biogenetic information <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/inm.12390?casa_token=d04HaZteTn8AAAAA%3A2VHaao4NggIeau1ElHswCScOMAwmu_eh68wKPfZ7ncTjLpb5o56_9166UeIRIHHQxMzIfHdDdWMFR9E\" rel=\"nofollow noopener\" target=\"_blank\">provided to participants.\u00ab<\/a>.\u00a0<\/p>\n<p>A look at the studies on the general public cited by <a href=\"https:\/\/www.cambridge.org\/core\/journals\/the-british-journal-of-psychiatry\/article\/biogenetic-explanations-and-public-acceptance-of-mental-illness-systematic-review-of-population-studies\/E6078EED07031B5828DB84FCDC90657F#article\" rel=\"nofollow noopener\" target=\"_blank\">the reviews<\/a> revealed a mix bag of definitions, ranging from \u2018brain disease\u2019 to \u2018bad nerves\u2019 (se also <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0277953613004164\" rel=\"nofollow noopener\" target=\"_blank\">this review<\/a>).\u00a0<\/p>\n<p>Crucially, none of the definitions explicitly stated the role of the environment in conjunction with genetics.<\/p>\n<p>In other words: People were not fully informed about what a \u2018biogenetic explanation of mental ill health\u2019 really means, i.e., that genetics contribute but do not wholly cause psychiatric illness.\u00a0<\/p>\n<p>Participants would have instead relied on their own interpretation which, given <a href=\"https:\/\/link.springer.com\/article\/10.1007\/S12687-018-0363-7\" rel=\"nofollow noopener\" target=\"_blank\">widespread misunderstanding of genetics<\/a> and the complexity of psychiatric genetics as a field, is likely to have been incorrect.\u00a0<\/p>\n<p>Such interpretations may have included the common-but-mistaken belief that an illness being \u2018genetic\u2019 means it is completely determined by your genes.\u00a0<\/p>\n<p>Therefore, it may be more correct to conclude that greater stigmatisation of psychiatric disorders (and antidepressants) is tied up with misunderstandings of biogenetic causes, rather than the biogenetic explanation in its true form \u2013 that genetics contributes to but always interacts and combines with environmental factors.<\/p>\n<p>Prejudice, self-blame, and guilt can be alleviated<\/p>\n<p>Indeed, research demonstrates that providing clear information about the role of both genetics and the environment in mental health conditions to members of the public <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/ajmg.b.33016\" rel=\"nofollow noopener\" target=\"_blank\">reduces prejudice towards psychiatric conditions<\/a>.<\/p>\n<p>Further, studies that provided psychiatric genetic counselling \u2013 a process through which a trained medical professional helps individuals understand the genetic and environmental contributions to their illness \u2013 have found such explanations <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/cge.12415?casa_token=I5KgKEEN2QIAAAAA%3AMkOYCnVdG2xfTEYg6mvzMWS5i7O3-wU3vkEHKp2bks9fFpB3S5xXabKhlej1Hb8R2c8vGok-d-A7Bnw\" rel=\"nofollow noopener\" target=\"_blank\">empower people<\/a> with mental illness and <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1002\/jgc4.1128\" rel=\"nofollow noopener\" target=\"_blank\">addresses internalised stigma<\/a>.\u00a0<\/p>\n<p>In other words, appreciating the role that genetics plays in one\u2019s illness facilitates a fresh perspective and a deeper acceptance towards one\u2019s condition.<\/p>\n<p>Understanding that genetics likely contributed to their mental health condition helped participants alleviate the feelings of blame and guilt that stemmed from believing their condition was entirely within their control.\u00a0<\/p>\n<p>This realisation fostered a sense of empowerment, not only by encouraging the adoption of protective strategies to improve mental well-being but also by reducing the shame associated with taking medication.\u00a0<\/p>\n<p>Ultimately, this new perspective offered a profound sense of relief and agency over their mental health, shifting the focus from self-blame to a more compassionate and informed view of their condition.<\/p>\n<p>So why are antidepressants still stigmatised in 2025?\u00a0<\/p>\n<p>All in all, it is likely antidepressants are stigmatised due to a toxic blend of misunderstandings and historical biases towards psychiatric illness.\u00a0<\/p>\n<p>First, we have the notion that people who take antidepressants might be severely ill or are \u2018weak\u2019 and unable to process what some people perceive as \u2018normal\u2019 emotional difficulties.\u00a0<\/p>\n<p>Then, we have the concern that taking medication \u2013 even for psychiatric illness \u2013 could lead to \u2018addiction\u2019.\u00a0<\/p>\n<p>Finally, antidepressants address neurotransmitter imbalances in your brain. The knock-on effect might be that people see depression as an illness largely or wholly caused by your biology.<\/p>\n<p>From the evidence above, we know that such beliefs are linked to seeing sufferers as more dangerous and as people to avoid. <\/p>\n<p>Mix all these things together and what do you get?
