On September 13, a traveler with measles passed through Philadelphia International Airport, moving between terminals over hearly five hours. Six days later, the city’s health department issued an exposure warning<\/a>.<\/p>\n

The warning concerned a single traveler. The public-health problem it illustrated was considerably larger.<\/p>\n

“Epidemiologically, this is news buzz,” Dr. Matthew Ferrari, director of the Center for Infectious Disease Dynamics at Penn State, told me. Operationally, though, it is something else. Airport exposures generate large numbers of people who develop routine fevers after passing through a terminal \u2014 contacts who must be tracked down, tested, and cleared, and who turn out, almost always, not to have measles. Each one costs time from a public health workforce already choosing where it can afford to respond. “For a response that is already strapped for resources,” Ferrari said, “this is a hit they shouldn’t have to take.”<\/p>\n

The Association of State and Territorial Health Officials<\/a> puts the average cost of a measles outbreak response at more than $750,000 \u2014 not counting broader societal costs such as lost productivity and caregiver time. One Michigan county health department<\/a> received $100,000 in emergency contact-tracing funds this spring and had “mostly used and allocated that already” by April.<\/p>\n

Pennsylvania has confirmed 890 measles cases<\/a> as of September 26, with four deaths \u2014 the state’s first measles fatalities in 35 years. Nationally, the United States has recorded 3,659 cases across 47 jurisdictions<\/a> as of September 24. For most of the quarter-century since measles was declared eliminated in 2000, outbreaks were relatively small and quickly contained. The resurgence of 2025 and 2026 has changed that: each major outbreak \u2014 in Texas, Utah, South Carolina, and Pennsylvania \u2014 has been larger and longer than anything in the post-elimination era.<\/p>\n

“The fact that we’ve had multiple of these certainly implies that we have the conditions for endemic persistence,” Ferrari said \u2014 the conditions, not yet the reality, under which measles circulates continuously in the United States rather than arriving in imported chains that public health can chase down.<\/p>\n

Measles is the hardest case in outbreak control. Containing it requires population-level immunity of around 95% \u2014 higher than almost any other common disease \u2014 and the outcome turns not just on the national average but on where the susceptible people are. Among American kindergartners, two-dose MMR coverage fell from 95.2% in 2019\u201320 to 92.4% in 2025\u201326<\/a>. That average already falls below the threshold, and it conceals a further danger: susceptibility is not distributed evenly. Where unvaccinated people cluster, outbreaks can take hold even when rates elsewhere remain high.<\/p>\n

The question this raises is no longer simply whether agencies can contain individual outbreaks. It is whether they can contain enough of them. “This is qualitatively different,” Ferrari said. For three decades, American outbreak response operated on the assumption that measles could be caught early and extinguished. That assumption has not been tested like this since elimination.<\/p>\n

Who Is Protected, And Who Isn’t<\/p>\n

Two MMR doses are about 97% effective; one dose offers 93% protection<\/a>. People born before 1957 are generally presumed immune<\/a>. Anyone who doesn’t know their vaccination history, or who received a single dose under the older pre-1989 schedule, should check their status. Infants, pregnant people without immunity, and people with compromised immune systems face particular risks.<\/p>\n

The monitoring window for the Philadelphia airport exposure runs through October 5. Symptoms \u2014 fever, runny nose, cough, red eyes, followed by the rash \u2014 appear seven to fourteen days after exposure. Call a doctor before going in person.<\/p>\n

The airport warning concerned one infected traveler on one evening. The larger question is what happens when such events stop being exceptional \u2014 when a system built to interrupt isolated chains of transmission must start deciding which investigations can wait.<\/p>\n

“The ugly part about prioritizing,” Ferrari said, “is that not everyone can be first.”<\/p>\n
