Her paychecks come out to roughly $2,160 every two weeks, after taxes, retirement contributions (about $130), union dues ($52), and other deductions. (A new union contract for residents is currently in negotiations after the previous one expired.) She has also gotten a stipend twice a year for living and educational expenses — most recently, it was $2,250.

Though her pay will improve after residency, “I didn’t ever imagine that as a doctor I would be living paycheck-to-paycheck, basically,” she said. Her patients at the safety-net hospital often assume she can’t relate to their financial strain.

“One time, a patient asked me how much I make, and I told him,” Torlone said. “He was really surprised. The plan when they moved to Boston was for him to take over the shop, but the couple “underestimated how demanding it would be,” Torlone said. Whether it happens now will be a matter of timing.

“That is the dream,” Torlone said, “but my husband is very kindly willing to put his aside for mine.”

The expenses: Torlone and her husband pay $2,600 a month for their 700-square-foot garden-level apartment, a one-bedroom with an office-turned-nursery. It was a stroke of luck finding landlords who “don’t believe in raising rent,” Torlone said, and the unit is blocks from the hospital, sparing her a commute. “Missing any more time during the day with my kids would be devastating to me,” she said.

<img decoding="async" id="img-R2BH6OBZRZ3FCEWPYPKTMWK7DI-image" alt="Chris Torlone moved a small table in her South End apartment. Car insurance costs about $187 a month; gas and tolls run about $250. Monthly utility bills for gas, electricity, and Wi-Fi, meanwhile, clock in around $200 to $390.

They try to minimize takeout and coffee (about $100 a month) and mostly cook at home. Buying food for a family of four is “nuts,” Torlone said, particularly because “my toddler eats what an adult eats.” Stop & Shop, Trader Joe’s, or Market Basket hauls run about $800 a month, with baby formula tacking on about $130 more. Free health coverage for her whole family (with minor copays).

The debt: But Torlone’s career choice also means being saddled with debt. Lots and lots of debt. “I owe the government, like, $420,000,” she said — all from med school, plus interest.

Torlone had been paying nothing under the SAVE repayment plan, but after that was struck down earlier this year, she enrolled in a different plan requiring monthly $275 monthly payments. “They’re only that little because I have two children,” she said.

If all goes to plan, she will make payments for the next decade or so, when her debt is erased under public service loan forgiveness — “if that still exists, dear God,” she said.

The child care: Until recently, “our childcare has been the grandmothers,” Torlone said, but “now that we have two under 2, it’s a lot for even a young person.”

So Torlone and her husband started looking into day care for their toddler, a spirited troublemaker who loves Curious George. Then they did the math.

“We were like, ‘Oh my God, we can’t even afford three days a week,’” Torlone said — $2,200 a month.

They debated whether her husband should stay home full time, but decided against it. She already hates that becoming a doctor means losing so much time with her kids. “If he stayed home,” she said, “I would get so jealous.”

Chris Torlone’s oldest child, who is nearly two years old, played with her stethoscope and badge.David L. Ryan/Globe Staff

Thankfully, a family member offered to chip in for day care costs, an idea Torlone might once have bristled at. “But I think my bar for accepting help has completely changed now that I have kids,” she said.

The savings: After they “emptied their savings” moving from Chicago to Boston, Torlone said, she and her husband have focused on rebuilding their nest egg. Because they sidestepped child care and student loan payments, they were able to squirrel about $30,000 into their shared savings account. (They also have separate accounts, but “it’s all kind of arbitrary,” Torlone said.) But with those costs now materializing, “less, if anything, will be going into savings,” Torlone said.

To put that money to work, Torlone is considering opening a CD account, which can provide a short-term boost. “If we’re gonna buy a house in the next three to five years,” Torlone said, “then we want to be able to access that money.”

The goals: Often, that goal of buying a home — in Melrose, perhaps, or on the North Shore — feels like a pipe dream, particularly when she dares to peruse Zillow. “I stopped looking because it made me sad,” she said.

She has heard about first-time homebuyer programs for physicians, which could offer a lower down payment requirement. Her parents have also offered to help, but she wants their money to be for them.

“I know they wouldn’t offer unless they wanted to,” she said. “But I also know that having kids, I would offer — I would literally do anything.”

Chris Torlone and her oldest son played with her stethoscope. David L. Ryan/Globe Staff

The splurges: By and large, Torlone and her husband “live like we are super broke,” she said. A typical splurge is watching television with mocha chip ice cream. “We [have] just always kind of been simple people who are happy with simple life,” she added.

But as someone “in the business of unpredicted tragedy,” she said, she knows there is a time and place to splash out.

There was a road trip with their oldest to Shenandoah National Park last year, which she estimates cost around $4,000, and a $1,500 family photo session in New Hampshire earlier this year.

“One day, we are gonna have a house, and I want a picture to put up on the mantle,” she said. “And then I tortured myself over it. In the car afterwards, I broke down crying, and I was like, ‘What have I done?’”

The future: As Torlone prepares to wrap up residency next year, she is wrestling with the question mark that comes after.

She sees two main paths. One is a fellowship in infectious disease — her dream specialty — which would have pay comparable to residency and similarly grueling hours, she said. The other is working as an attending physician, which she expects would offer a low-six-figure salary even part time.

The idea of flexible hours is particularly appealing, especially as she hopes to have a third child someday. But so much feels impossible to plan for.

“I logically know that I’m gonna make more money, and that my work hours are gonna go down, and that things are gonna get better,” she said. “I think I just have to have faith and suspend my anxiety for now.”

Chris Torlone is a third-year internal medicine resident at Boston Medical Center.David L.
