Scientists have revealed a striking paradox at the heart of physical activity and brain health, drawing on data from more than 4.2 million individuals across 30 countries to examine how different forms of exercise affect dementia risk.

It transpires that recreational pursuits such as walking, cycling, running, and swimming are associated with a significantly lower likelihood of developing dementia. Physically demanding occupations, by contrast, appeared to raise the risk.

The landmark systematic review, published in the Lancet Public Health, suggests that the circumstances under which people are active — whether by choice or by necessity — may matter considerably more than the sheer volume of movement itself.

The research was spearheaded by an international team based at the University of Southern California, with Natan Feter, a postdoctoral scholar and research associate, serving as lead author.

Their analysis encompassed 74 individual studies, making it an exceptionally comprehensive piece of work. Participants ranged from 45 to 93 years of age at the outset, with a median follow-up period of a decade.

Throughout that time, investigators tracked diagnoses of all-cause dementia, Alzheimer’s disease and vascular dementia.

Physical activity levels were gauged through self-reported questionnaires or objective measurements, with each study comparing more active individuals against less active reference groups.

Crucially, occupational physical activity was assessed solely among those still in employment, excluding retired or non-working participants from that particular analysis.

The figures revealed that those who exercised during their free time had a 24 per cent lower risk of dementia compared with the least active individuals. Those in physically demanding roles, meanwhile, faced a 20 per cent greater risk.

Dr Steven Allder, a consultant neurologist at Re: Cognition Health who was not involved in the research, offered an explanation for why voluntary exercise may be uniquely protective.

“Regular exercise improves cardiovascular health and blood flow, helps control blood pressure, blood sugar and cholesterol, and may support the brain’s ability to maintain healthy connections between nerve cells,” he told Medical News Today.

Leisure activities carry a distinctive advantage, he noted, adding: “Going for a walk with a friend, swimming, cycling or playing a sport can combine physical activity with social interaction, enjoyment and mental stimulation.”

Yet Dr Allder was emphatic that it would be misguided to conclude physically strenuous work directly causes cognitive decline. He believes the elevated risk rather reflects the broader conditions surrounding such employment.

“Someone in a physically demanding occupation may spend many hours standing, lifting, carrying or performing repetitive movements, often with limited control over the pace or intensity and not enough opportunity for recovery,” he explained.

Workplace hazards including chronic stress, shift patterns, and exposure to air and noise pollution — both independently linked to dementia — may also contribute. The researchers themselves pointed to heightened psychosocial stress and hazardous occupational exposures as plausible factors.

There is a compounding effect, too. “Someone who is physically exhausted after a demanding working day may have less time or energy for recreational exercise, social activities and other behaviours that support healthy ageing,” Dr Allder observed.

Both experts urged readers to interpret the findings with appropriate caution. Dr Allder stressed the importance of distinguishing correlation from causation. “Because this was an observational study, we cannot say that occupational physical activity causes dementia or that leisure exercise directly prevents it,” he warned.

“We cannot completely rule out residual confounding – that is, the possibility that some of the association we observed is explained by factors we did not measure or fully account for,” he said.

He highlighted that this caveat applies particularly to the occupational findings, since physically taxing jobs tend to coincide with wider social, economic, environmental and health-related disadvantages.

“Our findings should not be interpreted as showing that physical activity at work itself causes dementia, but rather that the broader context in which that activity occurs may be important,” Feter added.

The study’s authors argued that public health messaging must move beyond simply urging people to be more active and acknowledge that not all physical activity takes place under equivalent conditions.

“When promoting physical activity at the population level, we must ensure everyone has equal access to choice-led, safe, pleasant, and meaningful physical activity,” Feter said.

Dr Allder reinforced the point that nobody should be deterred from staying active, regardless of their occupation.

“Walking, cycling, swimming, dancing or simply being active with friends can all be valuable,” he said.

“Dementia prevention is likely to involve a combination of factors, including regular exercise, cardiovascular health, good sleep, social and mental engagement and addressing other modifiable risk factors, rather than any single behaviour in isolation.”
